A transition between hospital care and returning home

With hospitals discharging patients more quickly than ever, often with little or no rehabilitation, people need a place to go to regain their strength and receive quality rehabilitation care before returning home.

St. Barnabas Health System has gained a reputation as a top-notch provider of rehabilitation services. In fact, during 2001 alone, a majority of the patients at St. Barnabas Nursing Home and Valencia Woods Nursing Center were rehabilitation candidates who returned home within a short period of time.

St. Barnabas Health System employs a team of specialists dedicated to ensuring patients reach their potential. Practitioners include:

• Physical therapists
• Occupational therapists
• Speech therapists
• Recreational therapists

The therapists work with patients recovering from a variety of health problems, including:

• Fractures
• Strokes
• Neurological disorders
• Amputations
• Complications from disease

As an added service, once patients are well enough to return home, they can continue to work with their same therapist on an outpatient basis. Rehabilitation services are also available to community members on an outpatient basis.
